This facility is focused on short-term rehabilitation after a hospital stay with long-term stays accounting for less than 20% of the overall patient population.
Job Expectations:
Assist in planning, organizing, implementing, evaluating and directing the overall operation of the Social Service Department. The Social Worker will provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental and psychosocial well-being.

Requirements:
- Prefer 2 Years Social Work experience in a SNF or Hospital
- 4 year Degree
