HealthMark Rehabilitation is an inhouse therapy company, serving the Skilled Nursing Facility patients and residents of HMG facilities.

We’re currently seeking a Full-time & PRN Physical Therapist to evaluate and treat residents and short-term patients at our beautiful Crowley Nursing and Rehabilitation facility in Crowley, Texas.

Responsibilities include:

· Develop and implement the Physical Therapy plan of care for facility residents and short-term patients.

· Provide supervision, as needed, for staff Physical Therapy Assistants in delivery of the PT plans of care.

· Assist as needed with patient screenings for the need for Physical Therapy.

· Communicate progress and any barriers toward PT goals with interdisciplinary team members.

· Complete timely PT documentation in compliance with regulatory, licensing, and payor requirements.

· Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.

· Provide and document patient, family, and caregiver training regarding each PT patient’s specific plan of care and recommendations for follow up.

· Perform additional duties as required.

Qualifications include:

· PT degree from an accredited Physical Therapy program.

· Current Texas license in good standing as a Physical Therapist.

· Meet physical requirements to deliver PT responsibilities.

· Experience in a geriatric rehabilitation setting preferred.

· Successful completion of hiring process / background check.
