HealthMark Rehabilitation is an inhouse therapy company, serving the Skilled Nursing Facility patients and residents of HMG facilities.
We’re currently seeking a Full-time & PRN Physical Therapist to evaluate and treat residents and short-term patients at our beautiful Crowley Nursing and Rehabilitation facility in Crowley, Texas.

Responsibilities include:
· Develop and implement the Physical Therapy plan of care for facility residents and short-term patients.
· Provide supervision, as needed, for staff Physical Therapy Assistants in delivery of the PT plans of care.
· Assist as needed with patient screenings for the need for Physical Therapy.
· Communicate progress and any barriers toward PT goals with interdisciplinary team members.
· Complete timely PT documentation in compliance with regulatory, licensing, and payor requirements.
· Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
· Provide and document patient, family, and caregiver training regarding each PT patient’s specific plan of care and recommendations for follow up.
· Perform additional duties as required.
Qualifications include:
· PT degree from an accredited Physical Therapy program.
· Current Texas license in good standing as a Physical Therapist.
· Meet physical requirements to deliver PT responsibilities.
· Experience in a geriatric rehabilitation setting preferred.
· Successful completion of hiring process / background check.

***This Sign On Bonus is to be paid in 4 installments. The first payment of $1,000 will be made with the next payroll after the full-time employee as documented above has successfully completed 90 days of work without any disciplinary action. The second payment of $1,000 will be made with the payroll following the completion of 6 months of satisfactory work performance at full-time status. The third payment of $1,000 will be paid upon the successful completion of 9 months of full-time employment without any disciplinary action. The final payment of $2,000 will be paid upon the successful completion of one year of employment as a full-time employee, without any disciplinary action.
