SOCIAL SERVICES Assistant

Job Description:

Under the direction of the Social Services Director, will help with planning, organizing, implementing, the overall operation of the Social Service Department. The Social Services Designee will provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental and psycho-social well-being.

Job Responsibilities: Assist in the following

· Resident/Family/Physician Intervention

· Psycho-social Services

· Vision, Hearing, Dental, Podiatry Service Referrals

· Assessments & Care Planning

· Discharge Planning
