Methodist Transitional Care is seeking experienced and dedicated LVN or RN Charge Nurses to join our team!

A Resort Style Rehabilitation Facility, our facility is equipped for high-acuity patients, as well as those with multiple illnesses, injuries, or rehabilitative issues: 90% short-term transitional patients. This will cultivate a wide variety of skill sets for your career!

Are you a dedicated Licensed Vocational Nurse passionate about skilled and transitional care and looking for a progressive career in healthcare? As a Charge Nurse you will assess resident needs, develop individual care plans, administer nursing care, evaluate nursing care and supervise Certified Nursing Assistants (CNAs) in the delivery of nursing care.

Job Responsibilities

As a LVN/RN you will supervise the nursing unit and coordinate the effective use of personnel, supplies, equipment and facilities. Additional responsibilities include:

  • Reviewing staffing patterns and census of nursing units; reassigning personnel when necessary to ensure residents needs and budgetary controls are met
  • Managing and analyzing patient care following admission/readmission; conducting initial assessment for newly admitted or readmitted patients
  • Assisting in creating and implementing care plans that address individual residents’ needs
  • Administering medications, IV therapy, gastronomy tube management as well as some wound care
  • Monitoring residents and communicating changes to physicians
  • Ensuring appropriate documentation of resident care and maintaining medical records to ensure safe, efficient and therapeutic care to residents
  • Troubleshooting and investigating complaints, taking necessary action to resolve problems as necessary.

Additionally, you must have:

  • LVN or RN License, in good standing
  • 1 year of experience as a nurse preferred
  • Effective communication skills
  • Ability to organize, prioritize and maintain confidentiality and react decisively in emergencies
  • Exceptional supervisory skills, taking a proactive role to supervising and developing nursing team
  • Excellent training skills, ability to provide hands-on training to nursing staff
  • Good problem solving and independent thinking skills
  • Case management skills, comfort with client relations
